The Texas Board of Nursing (BON) has the jurisdiction to hear and decide cases involving the practice of professional nursing in Texas. Any RN or LVN found guilty of violating the state laws and regulations may be subjected to a disciplinary case if not properly defended by a nurse attorney. 

At the time of the initial incident, an RN was employed as a Registered Nurse at a hospital facility in Lewisville, Texas, and had been in that position for seven (7) months.

On or about January l, 2016, while employed as a Registered Nurse, the RN failed to document the behavioral event where the patient had an altercation with another patient, received Emergency Medication Injections three times, and required the use of behavioral restraints. The RN’s conduct created an incomplete medical record and was likely to injure the patient in that other caregivers lacked sufficient information on which to base the patient’s care.

On or about May 29, 2016, while employed as a Registered Nurse, the RN failed to transcribe physician ordered Motrin for a patient into the patient’s medical record, although she documented she notified the physician of the patient’s dental pain and received an order for Motrin. The RN’s conduct resulted in an incorrect medication order which was subsequently used by other nurses and was likely to injure the patient from medication administered without a physician’s order.

On or about May 29, 2016, while employed as a Registered Nurse, the RN failed to document pain assessments for the patient. The RN’s conduct created an incomplete medical record and was likely to injure the patient in that other caregivers lacked sufficient information on which to base the patient’s care.

On or about September 9, 2016, while employed as a Registered Nurse, the RN failed to appropriately transcribe physician ordered Motrin for a patient. The RN entered the medication to be scheduled for administration every 6 hours, rather than as needed for pain. The error was discovered by the nursing staff the next day. The RN’s conduct resulted in an incorrect medication order which was subsequently used by other nurses and was likely to injure the patient from medication administered without a physician’s order.

On or about September 9, 2016, while employed as a Registered Nurse, the RN failed to document vital signs, neuro signs, level of consciousness, orientation, and other significant clinical information related to Electroconvulsive Therapy (ECT) for a patient, after she returned to the unit following her ECT, as required per policy. The RN’s conduct created an incomplete medical record and was likely to injure the patient in that other caregivers lacked sufficient information on which to base the patient’s care.

In response, the RN admits that she may not have entered the order in a timely manner, although she entered the order in her nursing notes. Also, the RN reports that she wrote an order for on September 9, 2016, and on September 10, 2016, for Motrin. Lastly, the RN reports there are documents that are highly relevant that were not provided by Terrell State Hospital.

The RN failed to properly present and defend her case against the court. She was disciplined warning to suspension of her nursing license for violating Texas Board of Nursing regulation.
