The RN’s conduct created an inaccurate medical record and was likely to injure the patient in that subsequent caregivers would not have accurate and complete information on which to base their care decisions.

In response to the incident, the RN states all staff is in-serviced a minimum of once a year on documentation of intake, output, and vital sign measurements when said staff are delegated for medication administration. The RN further states staff were specifically trained in the patient’s specific dietary needs.

As a result, the RN was subjected to take disciplinary action from the Texas Board of Nursing. The RN was given the chance to defend her license but she failed due to lack of proper defense from a nurse attorney. Unfortunately, her license was disciplined.
