She states she documented pain medication effectiveness behind the MAR because there was not a separate pain management sheet to chart on. She states one patient had requested pain medications at times that were eligible as they were given 3.5 — 4 hours apart. She states the resident had come to the nursing cart each time on 3/8/18 and requested the pain medication. She also states all medications were given by herself and accepted by patients so no need for waste, therefore she states no medications, hydrocodone or otherwise, were misappropriated by herself. She states the patient received the medications per request for pain, as prescribed by the physicians,
