On or about December 11, 2019, the LVN failed to sign the CSAR count as the oncoming nurse. Her conduct created an incomplete Controlled Substance medication record for the shift.

Because of this, the LVN was summoned by the Texas Board of Nursing to give her a fair chance to defend her case. She was given the chance to defend herself at the Board.

The LVN was given the chance to defend against her case and present herself to the Texas Board of Nursing. But because she failed to attend the hearing or even a nurse attorney to defend her, the Texas Board of Nursing placed her under disciplinary action but without any suspension whatsoever.
